Electroestimulación transcutánea en la región sacra en pacientes con vejiga urinaria hiperactiva

INTRODUCCIÓN: La vejiga hiperactiva (VH) es la disfunción miccional más común en la edad pediátrica, con una prevalencia de 5-20% en niños de 5 a 10 años de edad. El tratamiento con anticolinérgicos produce tasas de éxito muy variables y con bajo apego por los efectos colaterales.
OBJETIVO: Conocer la evolución clínica con la terapia de electro estimulación transcutánea sacra (siglas en inglés: TENS) en pacientes con VH.
MATERIAL Y MÉTODOS: Estudio de cohorte retrospectivo de pacientes con diagnóstico de VH que recibieron tratamiento con TENS transcutánea en la región sacra.
RESULTADOS: Se incluyeron 19 pacientes con VH, 12 femeninos y 7 masculinos, con seguimiento de 12 a 32 meses. 13 de los 19 casos, recibieron tratamiento previo con anticolinérgicos y/o toxina botulínica tipo A. La polaquiuria y la incontinencia de urgencia diurna mejoraron en 100% y 87.5% respectivamente. El estreñimiento crónico se corrigió completamente en todos los casos con laxante y TENS. En la enuresis asociada a VH la respuesta al TENS fue baja.
CONCLUSIONES: Los síntomas de VH muestran buena respuesta a la TENS en región sacra, sin efectos colaterales indeseables. La combinación de un laxante con TENS, parece mejorar la respuesta al estreñimiento. La TENS debe considerarse una mejor opción que los anticolinérgicos en el manejo inicial de niños con VH pues carece de efectos colaterales. En la enuresis asociada a VH, la respuesta es baja con TENS. Protocolos con mayor número de casos son necesarios para confirmar los resultados aquí mostrados.
