Factores de mortalidad en prematuros menores de 34 semanas de gestación

Resumen

OBJETIVO: Determinar los factores asociados con mortalidad en recién nacidos prematuros iguales o menores de 34 semanas de gestación.

MATERIALES Y MÉTODOS: Estudio de cohorte, retrospectivo, efectuado en recién nacidos prematuros, iguales o menores de 34 semanas de gestación y peso de ±1500 g ingresados a cuidados intensivos neonatales. Se revisaron: edad gestacional, sexo, peso, Apgar al minuto y 5 minutos, puntaje en la escala CRIB II, reanimación, tipo de ventilación; variables maternas: edad, control prenatal, enfermedades durante el embarazo, administración de corticosteroides prenatales y la mortalidad neonatal. Se utilizó estadística descriptiva y se determinó el riesgo relativo, con IC95%.

RESULTADOS: La mortalidad global fue de 19.1% a 7 días de vida extrauterina, 55.5% correspondió al género femenino; el peso promedio fue 771.1 ± 189 g, el 88.8% correspondió a prematuros extremos. El Apgar al minuto de vida fue de 7 a 10 en el 25.5%, y a los 5 minutos fue menor de 7, también en 25%. La mortalidad aumentó con el puntaje de CRIB >10. La única variable de riesgo fue el puntaje de CRIB II. Solo el puntaje de CRIB II mayor de 10, que correspondió a 18.8, se asoció con mortalidad.

CONCLUSIONES: La mortalidad es inversamente proporcional al peso y la edad gestacional. El puntaje de CRIB II es un indicador oportuno para identificar el riesgo de mortalidad en recién nacidos prematuros menores de 34 semanas de gestación.
